Shivakumar Narayanan is Associate Professor of Medicine at the University of Maryland School of Medicine’s Institute of Human Virology. He serves as Co-Director of THRIVE, where he helps lead comprehensive HIV care and initiatives to improve engagement and retention in care, and as Director of the General Infectious Diseases inpatient service. His clinical and research interests focus on HIV, hepatitis C, and infectious complications of substance use. At the Health and Recovery Practice (HARP), he integrates HIV prevention, infectious disease screening, and hepatitis C treatment with addiction care to reach people who face barriers to traditional healthcare. He helped establish the University of Maryland’s OPAT program and continues to develop accessible, integrated models of care. He also teaches and mentors medical students, residents, and infectious diseases fellows.

Infectious diseases at the intersection of substance use, HIV, hepatitis C, and social barriers to care. My interests include integrated infectious diseases and addiction treatment, harm reduction, long-acting therapies, and interventions that improve linkage to care and treatment completion. I also investigate endocarditis, wound-related infections, and amyloid kidney disease among people who inject drugs, alongside the development of practical care pathways for resource-limited settings.

Principal Investigator for the CROWN study of long-acting cabotegravir and rilpivirine in people with HIV and viremia and for a Maryland Governor’s Office of Crime Prevention and Policy–funded project.

Previous funded work includes long-acting antimicrobials integrated with treatment for opioid use disorder, molecular diagnostic testing, and clinical trials of HIV and COVID-19 therapies.
