Biosketch

A clinician-scientist in the field of cerebrovascular disease and stroke, Dr. Lal launched the new field of cognitive-mobility function in carotid stenosis through his ground-breaking research and pioneering leadership in this field. He has fundamentally changed our understanding of the range of morbidity associated with carotid atherosclerosis, well beyond stroke. His collective work over a decade has made sustained and transformational changes to our understanding of cerebrovascular disease. His current research is an integral part of the stroke-prevention drive of the National Institute of Neurological Disorders and Stroke, NIH, and is focused on how the structure and biomechanics of atherosclerotic plaque in the carotid artery affects brain function, and impacts plaque disruption, stroke, cognitive function and mobility function; and how these adverse outcomes respond to interventions such as pharmacotherapy, rehabilitation and surgical revascularization.

As co-Principal Investigator of the largest randomized trial in stroke-prevention funded by the NINDS-NIH, Dr Lal will be instrumental in determining the optimal treatment approach for over 10 million individuals in the United States with carotid atherosclerosis, and many more world-wide. As Principal Investigator of the National Carotid Stenting Registry, with its administrative center here at the School of Medicine, he is working with the NIH and the Centers for Medicare and Medicaid, to establish national physician credentialing, clinical performance and reimbursement standards for carotid artery stenting. As Director of the Center for Vascular Diagnostics, he manages one of the largest clinical vascular laboratories in the country with over 15,000 tests performed every year. The center adjudicates imaging data from several NIA, NINDS, DOD and the VA funded studies, during which he has developed several novel diagnostic vascular tests for vascular disease.

From 2004 to date, Dr. Lal has been a highly cited investigator worldwide in the field of carotid atherosclerosis. He has authored more than 125 publications including the Society for Vascular Surgery guidelines for carotid occlusive disease management. He serves on the Advisory Board of JCAHO for Stroke Center policy development, and is the President of the national venous disease society. He has also led numerous collaborations with investigators in vascular disease epidemiology, mechanisms, diagnostics and therapeutics.

This study examines the effects of intensive therapy for periodontitis (PD) to prevent the progression of cognitive impairment to dementia in high-risk older people with asymptomatic carotid stenosis. Results could highlight PD as a readily modifiable risk factor for dementia and inform a larger clinical trial to prevent dementia.

The major goals of this project are to gather data on physical function in patients with PAD who are receiving conservative treatment as well as those who have undergone an endovascular intervention for lower extremity arterial disease. We also seek to gather data on muscle perfusion while walking in PAD patients using ultrasound imaging with stabilized microbubbles.

The major goals of this project are to 1) review and adjudicate imaging studies performed in CREST-H (MR angiograms, MR brain perfusion scans and MR brain scans) 2) credential vascular ultrasound technologists at the >150 CREST-2 clinical centers, and 3) provide infrastructure to receive and archive all images obtained in the CREST-H trial. The major goals of CREST-H is to determine whether cognition can be improved by revascularization among a subset of CREST-2 patients with hemodynamic impairment at baseline.

The major goal of this project is to perform a systematic, adequately powered study to measure the magnitude of cognitive improvement in asymptomatic carotid stenosis patients undergoing carotid revascularization, its impact on quality of life, and its potential pathophysiological mechanisms.

The major goals of this project are to assess whether intensive medical management is superior to CEA and to CAS with respect to a composite end-point of stroke and death at 4 years follow-up. Secondary assessments will assess if the CEA or CAS vs. intensive medical management difference is affected by age, sex, severity or carotid stenosis, restenosis, or the level of control of cardiovascular risk factors, and whether recurrent stenosis after CAS or CEA is significantly higher than stenosis progression during intensive medical management.
