It is imperative that you check your email and respond to action items from the fellowship program, and various hospital entities such as the GME Office, Medical Staff Office, Human Resources, Employee Health, and UMMC related to your credentialing, onboarding, and systems/application access.
Orientation schedules will be emailed once they are finalized in June.

Electronic Credentialing Application - COMPLETED

- Three required forms under "Documents to Sign" must be completed via DocuSign.

EMPLOYEE HEALTH - TIME SENSITIVE DUE MAY 1, 2026
Phenom (HR Onboarding – Required First)
Before you can complete your Employee Health requirements, you must first complete HR onboarding in Phenom, UMMC’s onboarding system. If you have questions accessing or using Phenom, please email phenomsupport@umm.edu
- Welcome Email - you have received a welcome email containing a private link to complete your onboarding application in Phenom.
- Complete the Phenom Application - this application initiates your HR onboarding and collects basic demographic information, work authorization details, and conflict of interest disclosures.
- Acknowledgement Form - after submitting the application, you will receive an Acknowledgement Form confirming that your pre-employment onboarding has begun.
- HR Pre-Hire Essentials - once the Acknowledgement Form is processed, you will receive another onboarding email with access to the secure pre-employment onboarding portal, where you will complete the HR Pre-Hire Essentials Tour (online) and other required HR documents.
- I-9 Verification – after completing the Acknowledgement Form & Pre-Hire Essentials tour, a Talent Acquisition team member will contact you to schedule a virtual Webex I-9 document verification appointment. If you have not been contacted, please email the Education Office (anesresfellcoor@som.umaryland.edu )
- Employee Record Creation – completion of all HR onboarding documents generates your employee record and employee ID and grants you access to MyCority, where you will begin the Employee Health onboarding process and complete your pre-placement health evaluation.
Employee Health Pre-Placement Evaluation
The Employee Health evaluation is completed remotely and does not require an on-site appointment.
- If you are in the Baltimore area and prefer an on-site appointment, email your request to newhireummcdtc@umm.edu.
The remote evaluation includes completing required online questionnaires and uploading immunization records (e.g., MMR, Varicella, Tdap, etc.) in MyCority.
- Please note: Failure to submit or complete required vaccination records, TB skin test results, and the Treating Physician PrePlacement Medical Review form (if applicable) may result in a delay to your start date.
Accessing MyCority
You must complete your Phenom onboarding before accessing MyCority. Access to MyCority becomes active 24 hours after Phenom registration is completed. Please use MyCority for completion of online documents. Please do not send any of these documents to Employee Health or the Education Office via email or fax.

- Select “Forgot Password” to create your password

Drug Screening & Lab Work
- A urine drug screen (and possibly additional lab work) is required.
- Testing must be completed at a Quest Diagnostics Lab within the United States.
- EHS will email you a Quest requisition.
- Once received, you have 7 days to complete the urine drug screen at a Quest location of your choice.

Fit Testing
EHS will provide medical clearance for fit testing.
- You will schedule your fit testing appointment once you are onsite for orientation (Fit Testing Appointment Request Form).
- You will receive instructions on how to complete this step from EHS once you’ve submitted all your required documentation.
Please note: Medical clearance by EHS is required before you can begin work and receive pay. Failure to complete required items by the stated deadlines may delay your start date. Please do not contact Employee Health to confirm your clearance status; the Education Office is notified of any outstanding tasks and will follow up with you directly if needed.

MARYLAND ELECTRONIC DEATH REGISTRATION SYSTEM (MD-EDRS)
During the first week of July, MD-EDRS (Maryland Electronic Death Registration System) accounts will be created for new trainees.
You will receive two separate emails from EDRS:
- One email with your username
- One email with a time-sensitive activation link to create your password
The activation link expires after 24 hours, so please monitor your inbox and complete activation as soon as you receive the emails.
Please note: The emails will come from identity@mymdthink.maryland.gov and will be sent to your UMMS work email, not your personal email.

After logging into MD-EDRS, use the “Help References” tab for step-by-step guides and training materials, including an MD-EDRS Tip Sheet, Medical Certifier Training Guide, & Instructions for New MD-EDRS Users.
Password Attempts & Lockout Policy: After 5 failed login attempts, your account will be locked.
To avoid being locked out:
- After 2 failed attempts, select “Forgot Password” to reset your password
- A reset link will be sent to the same email address where your username was originally sent
If you are locked out:
- During normal business hours (M-F, 8am - 4pm):
- Click “Forgot Password” and enter your correct username and associated email
- Contact Theresa Boyd at Theresaboyd@umm.edu
- After hours (M-F, 4pm – 8am, weekends, holidays) or if your username/email is unknown:
- Locate another provider on your unit who can log in and have them complete the death certificate
- When prompted for “Who is this completed by?”, enter the name of the provider supplying the information
- The certificate will show as attested by the logged-in provider, which is acceptable
- If no colleague is available:
- Contact MD State EDRS Help Desk (EDRS.help@umaryland.gov | 410-764-4671), response within 4hrs
- You may also contact UMMC HIM Senior Managers (410-328-6750)
- Locate another provider on your unit who can log in and have them complete the death certificate
ORP ENROLLMENT (Required)
Maryland Medicaid requires all residents to enroll as a fully participating Medicaid provider under the Ordering, Referring, and Prescribing (ORP) Provider designation (residents fall under ORP).

EPIC Training
Portfolio EPIC Resident Ambulatory/Inpatient Training (completed in UMMS U)
This training consists of self-directed modules assigned in UMMS U. A test-out option is available.
Test‑out enrollments are expected to be sent via email approximately two weeks prior to your start date.
You will receive an email from PortfolioTraining@umm.edu with the subject line “ACTION REQUIRED – Portfolio Epic Test Out, Important Requirements and Instructions.”
You are allotted one hour to complete the test, and you will be able to view your test-out results after completion.
Test‑out results are determined as follows:
Pass (≥80%): You do not need to complete the modules prior to the virtual training.
Fail (<80%): You must complete all assigned modules prior to the virtual training (estimated 5 hours, self-paced).
Portfolio EPIC Provider Training (built into orientation schedule)
- Training date JULY 6, 2026
- Virtual, instructor-led, completed on your individual personal computer.
- A test-out option is not offered for this course.
IHI Open Training
UMMC IHI Open School Training Requirements
The Graduate Medical Education Committee has approved required Patient Safety and Quality Improvement (PSQI) training for all NEW residents joining UMMC in the upcoming academic year. This training is mandatory and is designed to supplement your specialty or subspecialty specific PSQI activities.
