When performing the NIH Stroke Scale, keep the following conventions in mind:


-- Administer scale items in their exact order.
-- Avoid coaching the patient.
-- Accept the patient's first effort.
-- Be consistent.
-- Score only what the patient actually does.
-- Include all deficits in scoring.
