Title: Tenecteplase Before Thrombectomy at 4.5 to 24 Hours for Basilar Artery Occlusion (ATTENTION LATE)

Category: Critical Care

Keywords: basilar artery occlusion, (PubMed Search)

Posted: 10/13/2026 by Quincy Tran, MD, PhD (Updated: 10/19/2026)

The BRIDGE-TNK trial in 2025 showed that Tenecteplase AND thrombectomy offered better functional outcome for patients with ischemic stroke from large vessel occlusion.

However, the ATTENTION LATE trial (published in September 2026) showed that for patients with basilar artery occlusion, routine administration of Tenecteplase before Thrombectomy did not improve functional outcome, when compared with thrombectomy alone, between 4.5-24 hour window, while rates of symptomatic intracranial hemorrhage were the same in both groups.
