Applications for the Sports Medicine Fellowship Program will be accepted through the Electronic Residency Application Service (ERAS).
Please include the following:
- Copy of medical school dean's letter
- Three letters of recommendation, including one letter from the program director
- Current Curriculum Vitae
- Personal statement
- Copy of medical school transcript
- Copy of USMLE transcript or equivalent
The application deadline is November 1st.
