Jury Duty

If a student receives jury summons while registered as a medical student at UMSOM, OSA can assist you.

  • Scan and send your jury summons ASAP to OSA.
  • OSA will draft a reschedule request. Please note, this request does not guarantee you will be excused from Jury Duty.
  • OSA will inform you once the request has been mailed and/or faxed.
  •  It is the responsibility of the student to confirm that the jury office has received the request.

Other Forms

If approval or a signature of a Dean is needed, please reach out to OSA.

VA/GI Bill® Educational Benefits

VA/GI Bill® Educational Benefits

Review VA Benefits, complete the VA Declaration of Intent and return to OSA with supporting documentation.

All new and continuing veteran and dependent students who are eligible for and intending to use educational benefits are REQUIRED to print, fill out, and submit the VA Enrollment Certification form each semester after you have enrolled for classes.

Upon receipt of your Enrollment Certification form, the School of Medicine’s SCO (School Certifying Official) will certify your enrollment to the VA using the Veterans Enrollment Management system. Once enrollment is certified, your tuition is paid directly to Student Accounting. In accordance with federal regulations, GI Bill® students should not be imposed a late fee, and therefore, a list of GI Bill® students is reported to Student Accounting to prevent penalty.

GI Bill® is a registered trademark of the U.S. Department of Veterans Affairs (VA). More information about education benefits offered by VA is available at the official U.S. government Web site at Benefits.va.gov.
