State Board Forms

The Office of Student Affairs will verify medical education for state boards only.

Please send the form through one of the following methods:

If the State Board requires a transcript to go along with your state board certification, please send all documents to the Office of the Registrar. They will forward the documents, with the transcript, to our office for processing and mailing.

All other requests can be forwarded to the National Student Clearinghouse:

When requesting degree certifications through the National Student Clearinghouse, you must use University of Maryland, Baltimore. 

Transcripts

The Office of Student Affairs does not process transcripts. Transcript requests can be sent to the Office of the Registrar.

Residency and Employment Verification

The Office of Student Affairs does not verify residencies at the University of Maryland Medical Center or employment. Please forward those requests to the Human Resources Service Center to the attention of Cathleen Flack.
