Application Requirements

Immunization Requirements

A strict immunization policy is enforced at UoMSOM. Compliance with immunization documentation for VSLO students mirrors UoMSOM students. All VSLO students must complete and submit AAMC Standardized Immunization Form prior to being approved to participate in any elective. Students who fail to document the following requirements will be delayed:

  1. Tetanus/Diphtheria - booster within 10 years.

  2. Measles - Students born after December 31, 1956 must show proof of either two immunizations with live measles vaccine on or after the first birthday or evidence of immunity (antibody titer). Immunization dates must include month/day/year.

  3. Rubella - proof of immunization or antibody titer. Immunization dates must include month/day/year.

  4. Mumps - proof of immunization or antibody titer. Immunization dates must include month/day/year.

  5. Varicella (Chickenpox) - proof of immunization, documented history of illness, or antibody titer.

  6. Tuberculin skin test (TST) for the purpose of testing for Tuberculosis: Visiting students must provide documentation of two TB skin tests (TST) administered and interpreted within 12 months of the requested rotation. The most recent TST should be administered and interpreted within 3 months of the rotation. Students who cannot provide evidence of two TST's within 12 months of the rotation, must have a 2-Step TB skin test administered. Placement of consecutive TB skin tests with one-to-three (1-3) weeks of each other are required. If either the first or second TB skin test is positive, documentation of a negative chest X-ray in the form of a written chest x-ray report and where applicable, a written report of INH treatment of prophylaxis therapy is required. Additionally, a Student Health TB Questionnaire must be completed by any student who has had a positive TST.

  7. Hepatitis B - Hepatitis B vaccine series (3) or positive antibody screen.

  8. Seasonal influenza vaccination within twelve (12) months of the rotation.

Liability/Malpractice Insurance Requirement

Students must have a minimum of $1,000,000/$3,000,000 liability/malpractice insurance coverage. Documentation with dollar amounts and expiration date is required. 

Health Insurance Requirement

Each VSLO student must upload copy of their current personal health insurance card to the supplemental document section of their application.
