How to Apply

‌Applications for the Sports Medicine Fellowship Program will be accepted through the Electronic Residency Application Service (ERAS).

Please include the following:

• Copy of medical school dean's letter
• Three letters of recommendation, including one letter from the program director
• Current Curriculum Vitae
• Personal statement
• Copy of medical school transcript
• Copy of USMLE transcript or equivalent

The application deadline is November 1st.
