Thank you for participating in our research project to asses the outcome of anti-reflux surgery from a patient perspective. The results that we are collecting are completely confidential and are being used to monitor long term patient satisfaction after surgery. You were asked to participate in the survey because you have been evaluated in the Aerodigestive Center for reflux disease and are considering or have completed anti-reflux surgery. The link below will open a survey which measures the degree of symptoms you are experiencing, and we will ask you to complete this survey before and after your operation. The survey will be repeated 1, 3, 6, and 12 months after your operation, and then yearly from that point on.

The survey consists of 66 questions and should take less than 10 minutes to complete. Thank you again for your participation.
