Community Health Awareness and Monitoring Program (C.H.A.M.P.)
C.H.A.M.P. is a community based health education and prevention program. Its mission is to place behavior change strategies in the hands of people who can motivate and assist one another to adopt healthier lifestyles and become self-reliant.
Community volunteers are trained and certified to provide screening, referrals, monitoring and culturally sensitive health education to members of the community. Programs include:
- "I Can..I Will ... Lose Weight Program",
- "Blood Pressure Provider Course",
- "Community Advocates for Diabetes Education",
- "Community Advocates for Cholesterol Education",
- "Community Advocates for Smoking Cessation",
- "Smoking Cessation" and
- "Healthy Kids, Healthy Futures".
